1. Brand name of Grandaxin 50mg and its mechanism of action

Grandaxin 50mg contains the active ingredient tofisopam, which is known for its mild anti-anxiety effects. It is a short-acting medication with a broad therapeutic range. The exact mechanism by which it works is not fully understood. However, tofisopam differs from typical 1,4-benzodiazepine drugs in both its chemical structure and its pharmacological properties. This difference makes Grandaxin effective in treating anxiety, especially when it is accompanied by physical symptoms like fatigue or a sense of emotional numbness.

What other effects does Grandaxin 50mg have? Unlike traditional benzodiazepines, it does not cause regular sleepiness. The drug does not have muscle-relaxing or anticonvulsant properties. Additionally, Grandaxin 50mg does not negatively impact a patient’s intellectual or physical abilities. However, it has a mild stimulating effect. The toxicity of tofisopam is quite low, and side effects are usually mild. Also, long-term use does not cause physical or mental dependence, making it safer for extended treatment.

Regarding how tofisopam works in Grandaxin 50mg, it is quickly absorbed into the body through the digestive system. The highest concentration of the active ingredient in the blood is usually reached 1 to 1.5 hours after taking the drug. About 50% of tofisopam binds to plasma proteins. Once absorbed, the drug is mainly broken down in the liver by a process called demethylation. Around 60% of the drug is then excreted through urine, and 40% is eliminated in feces. The drug’s half-life, which is the time it takes for half of the drug to leave the body, ranges from 6 to 8 hours.

2. Uses and dosage of Grandaxin 50mg

Grandaxin 50mg comes in the form of white or slightly grayish round tablets with a flat surface and a cut edge. Each tablet contains 50mg of tofisopam along with other ingredients like potato starch, gelatin, talc, and stearic acid. Doctors prescribe Grandaxin 50mg for the following conditions:

  • It helps treat symptoms such as anxiety, autonomic dysfunction (like heart palpitations or sweating), lack of energy or motivation, emotional numbness, fatigue, or even signs of depression.
  • It can also be used for non-cardiac chest pain (often referred to as angina).
  • In cases of alcohol withdrawal, Grandaxin 50mg helps reduce physical symptoms like tremors and agitation, particularly in early stages of delirium.

In addition, Grandaxin 50mg can be used when muscle relaxants are not recommended, especially in conditions like muscle weakness, muscle diseases, or nerve-related muscle wasting.

For adults, the typical dosage is 1 to 2 tablets per dose, up to 3 times a day, with a maximum dose of 300mg per day. For occasional use, 1 to 2 tablets may be sufficient.

When treating an illness, doctors will prescribe a dose that suits the specific condition. Usually, there is no need to gradually increase the dose since Grandaxin 50mg is well tolerated and does not affect alertness or mental clarity when taken as directed.

3. Possible side effects when using Grandaxin 50mg

While taking Grandaxin 50mg, some people may experience mild and temporary side effects. However, if the drug is taken in higher doses, side effects may be more noticeable.

Gastrointestinal side effects: These include symptoms like loss of appetite, constipation, bloating, nausea, dry mouth, and in some cases, jaundice (yellowing of the skin and eyes) caused by bile buildup in the body.

Central nervous system side effects: Some people may experience headaches, feelings of nervousness, difficulty sleeping (insomnia), irritability, or restlessness. In rare cases, confusion or seizures may occur, especially in people with a history of epilepsy.

A rash, sometimes itchy, or heat rashes can appear on the skin. In very rare cases, there may be trouble breathing (respiratory depression).

When taking Grandaxin 50mg, there are a few things to keep in mind to ensure safe use:

People who allergic to Grandaxin or any other component of the medicine, people with severe breathing problems or a history of sleep apnea (when breathing stops for short periods during sleep) should avoid this medication, patients who have had a history of coma

Pregnant women should avoid using Grandaxin during the first three months of pregnancy, and it should also be avoided while breastfeeding.

Grandaxin should be used with caution in elderly people, or those with developmental delays, or problems with their kidneys or liver. These groups may be more likely to experience side effects.

Grandaxin with substances that can affect the brain and nervous system, such as alcohol, certain antidepressants, allergy medications, sleeping pills, or painkillers similar to opiates, as it could increase side effects.

Grandaxin should not be used for patients with long-term mental health disorders, fears, or obsessive thoughts because it may increase the risk of self-harm or cause agitation.

People with brain conditions like arteriosclerosis (hardening of the arteries in the brain) may be more likely to experience seizures when using Grandaxin.

Taking too much Grandaxin can affect the nervous system, causing problems like vomiting, confusion, coma, or difficulty breathing. If overdose occurs, do not try to make the person vomit if their brain is seriously affected. Instead, doctors may use treatments like activated charcoal or a stomach pump to reduce the drug’s absorption. Flumazenil (Anexate) can reverse some of the effects but should not be used in large amounts. People who are allergic to benzodiazepines or flumazenil should avoid these treatments.

If a patient’s blood pressure drops too low, fluids can be given intravenously, and the patient should lie in a specific position to help increase blood flow. If that doesn’t work, medications like dopamine or noradrenaline may be used to bring blood pressure back to normal.
