Success treatment of neonatal respiratory failure case at Vinmec Phu Quoc using new method

Vinmec Phu Quoc General Hospital has successfully treated a case of respiratory failure due to delayed pulmonary drainage in infants with surfactant pump therapy using Lisa - the latest method in the world. On the afternoon of May 20, 2020, Vinmec Phu Quoc International General Hospital welcomed baby Khai Uy, weighing 3,320 grams at birth. However, 1 hour after birth, the baby started showing abnormal signs of being cyanotic, moaning, and chest indrawing. Based on the clinical signs, the doctors determined that the patient had respiratory failure due to delayed clearance of pulmonary fluid. After 24 hours of continuous breathing through a nasal cannula with positive pressure ventilation, the patient's condition still did not improve.

The results of the chest X-ray were not positive. After consultation, the doctors decided that it was necessary to perform a surfactant pump on the patient immediately to avoid causing serious complications. The question then was whether to inject surfactant through the endotracheal tube or to follow the new Lisa method.

If pumping using the old method, the patient would need sedation, intubation and invasive mechanical ventilation, and parenteral feeding. That would expose the baby to a number of risks such as hypotension, decreased cerebral perfusion, pneumothorax, pulmonary bleeding, difficulty digesting breast milk, and enteritis later in life.

However, the Lisa method can solve the above problems. After remote consultation with pediatricians throughout the Vinmec system, the doctors decided to choose to perform a surfactant pump using the new Lisa method. Limited conditions on the island and the fact that Lisa was a new method were two great challenges for the medical team at Vinmec Phu Quoc. During the treatment, the patient did not need mechanical ventilation and was able to eat by mouth soon later. After 4 days, the baby was able to enjoy breastfeeding and cry loudly.
