For the convenience of medical examination and treatment at Vinmec, we would like to send you detailed instructions as below. Please read the instructions carefully and prepare the relevant paperwork to ensure that the medical examination and treatment process goes smoothly.
1. Necessary documents
If this is your first visit at Vinmec, please come to the Reception Desk at the Outpatient Department to open your visit file. The information needs to be provided:
- Full name
- Passport or Birth Certificate (for children)
- Insurance card (if any) for claim procedure
- Cards/ Vouchers (if any)
Notice:
- The above-mentioned documents need to be brought with you every time using services at Vinmec.
- Before your consultation or performing any medical intervention, you are identified by at least 2 parameters: (1) Full name, (2) Date of birth and PID code (if needed) to optimally avoid any possible errors.

2. Outpatient examination procedure at Vinmec
- Step 1: Fill up the register form (1 copy) and Copy of identification documents, insurance card...
- Step 2: Sign the payment documents (02 lists of services + 1 payment receipt); Sign the insurance payment (01 if with private insurance)
- Step 3: Consent to HIV testing, consent to treatment before using the services (if any) or refusal of treatment (1 copy)
- Step 4: Sign the payment documents (02 lists of services + 1 payment receipt); Sign the insurance payment (01 if with private insurance)
- Step 5: Sign medical service request form (2 copies)
- Step 6: Sign medical service request form (2 copies); Sign consent to treatment (1 copy)
- Step 7: Sign payment documents (2 lists of services + 1 payment receipt); Sign insurance payment and payment commitment (2 copies for patient has insurance)
- Step 8: Sign check-in and check-out confirmation documents (1 copy); Sign payment documents (2 lists of services + 1 payment receipt); Sign compensation insurance verification (1 copy for patient uses insurance)
