1. What is Nexium mups 40mg, 20mg?
Nexium Mups is a product of
AstraZeneca (Sweden). The drug is prepared in the form of gastro-resistant film-coated tablets with the main active ingredient being Esomeprazole 40mg which is used to treat diseases such as gastroesophageal reflux disease (GERD), peptic ulcers, and Zollinger-Ellison syndrome.
Nexium mups with the main active ingredient Esomeprazole, are prepared in the form of gastro-resistant tablets with 2 doses of 40mg and 20mg, indicated in the following cases:
Adults
Treatment of gastroesophageal reflux disease (GERD):
- Treatment of reflux esophagitis.
- Long-term treatment for patients with healed esophagitis to prevent recurrence.
- Symptomatic treatment of gastroesophageal reflux disease (GERD).
Combined in the treatment regimen for duodenal ulcers caused by Helicobacter pylori:
- Healing duodenal ulcers infected with Helicobacter pylori.
- Prevention of recurrence of peptic ulcers in patients with ulcers infected with Helicobacter pylori.
Patients requiring continuous treatment with non-steroidal anti-inflammatory drugs (NSAIDs):
- Healing gastric ulcers caused by NSAIDs.
- Preventing gastric and duodenal ulcers caused by NSAIDs in at-risk patients.
- Long-term treatment after intravenous treatment to prevent rebleeding of peptic ulcers.
Treatment of Zollinger Ellison syndrome.
Adolescents aged 12 years and older.
Treatment of gastroesophageal reflux disease (GERD):
- Treatment of erosive reflux esophagitis.
- Long-term treatment of patients with healed esophagitis to prevent recurrence.
- Symptomatic treatment of gastroesophageal reflux disease (GERD).
- In combination with antibiotics in the treatment of duodenal ulcers caused by Helicobacter pylori.
2. Mechanism of action of Nexium mups 40mg, 20mg
Esomeprazole is a weak base, concentrated and converted to the active form in the highly acidic environment of the secretory canaliculi of the parietal cell, where it inhibits the H+K+-ATPase pump and inhibits both basal and stimulated secretion.
3. How to use Nexium mups 40mg, 20mg
3.1. How to use
Nexium Mups should be swallowed whole with liquid, do not chew or crush the tablet.
If the patient has dysphagia, the tablet can be dispersed in half a glass of non-carbonated water (other liquids should not be used because they can damage the coating that helps the drug dissolve in the intestine). Stir until the tablet is completely dispersed and drink the dispersion containing the microparticles within 30 minutes. Rinse the glass with half a glass of water and drink. Do not chew or crush the microparticles.
For patients who cannot swallow, the tablet can be dispersed in non-carbonated water and administered through a nasogastric sonde.
3.2 Dosage of Nexium mups 40mg
Dosage in the treatment of gastroesophageal reflux disease (GERD):
- Treatment of reflux esophagitis: 40 mg, 1 daily for 4 weeks. Treatment may be continued for 4 weeks if symptoms or esophagitis persist.
- Long-term treatment for patients with healed esophagitis to prevent recurrence: 20 mg, 1 daily.
- Symptomatic treatment of gastroesophageal reflux disease (GERD): 20 mg, 1 daily.
Dosage in combination with some Helicobacter pylori eradication regimens: Nexium Mups 20-40 mg/time, 2 daily.
Patients requiring continuous treatment with non-steroidal anti-inflammatory drugs (NSAIDs):
- Healing of gastric ulcers caused by NSAIDs: usual dose 20 mg, 1 daily. Treatment duration is 4-8 weeks.
- Prevention of gastric and duodenal ulcers caused by NSAIDs in patients at risk: 20 mg, 1 daily.
Dosage in case of prolonged treatment after intravenous treatment to prevent rebleeding of peptic ulcers: 40 mg, 1 daily for 4 weeks.
Dosage in case of treatment of Zollinger-Ellison syndrome:
- The recommended starting dose is 40 mg, 2 daily. Then adjust according to the response of each patient.
- Clinical data show that most patients are controlled with a dose of 80 - 160 mg/day.
- When the daily dose is greater than 80 mg, the dose should be divided into 2 times/day.

4. In which cases should Nexium mups 40mg, or 20mg not be used?
Do not use Nexium Mups 40mg, 20mg in the following cases:
- History of hypersensitivity to esomeprazole, benzimidazoles, or other ingredients in the formulation.
- Esomeprazole should not be used concurrently with nelfinavir, or atazanavir.
5. Precautions when using Nexium mups 40mg, 20mg
When there are any alarming symptoms such as unintentional weight loss, recurrent vomiting, dysphagia, vomiting blood, or black stools, and when gastric ulcer is suspected or present, malignancy should be excluded because treatment with Nexium Mups may relieve symptoms and delay diagnosis.
Esomeprazole, other antacids, may reduce the absorption of vitamin B12.
Severe hypomagnesemia has been reported in patients treated with proton pump inhibitors for a minimum of 3 months, with most cases lasting up to 1 year.
Proton pump inhibitors, especially when used in high doses and over long periods (>1 year), may modestly increase the risk of hip, wrist, and spine fracture, especially in the elderly or in the presence of other risk factors.
6. Side effects of nexium mups 40mg, 20mg
When using Nexium Mups, you may experience undesirable effects (ADR).
Common, ADR > 1/100
- Nervous system: Headache.
- Gastrointestinal: Abdominal pain, constipation, diarrhea, flatulence, nausea/vomiting.
Uncommon, 1/1000< ADR < 1/100
- Metabolic, nutritional: Peripheral edema.
- Psychiatric: Insomnia.
- Nervous system: Dizziness, paresthesia, somnolence.
- Ear and labyrinth: Vertigo.
- Gastrointestinal system: Xerostomia.
- Skin, subcutaneous tissue: Dermatitis, pruritus, rash, urticaria.
- Musculoskeletal system, connective tissue: Fracture of hip, wrist, spine.
Rare/very rare, ADR < 1/1000
- Hematologic system: Leukopenia, thrombocytopenia, agranulocytosis, pancytopenia.
- Immune system: Hypersensitivity reactions such as fever, angioedema, anaphylactic reaction/anaphylactic shock.
- Metabolic system, nutrition: Hyponatremia.
- Psychiatric system: Agitation, confusion, depression, irritability, hallucinations
- Nervous system: Taste disturbance.
- Eye: Blurred vision.
- Respiratory system: Bronchospasm.
- Skin, subcutaneous tissue: Alopecia, photosensitivity, erythema multiforme, Stevens-Johnson syndrome, toxic epidermal necrolysis.
- Musculoskeletal system, connective tissue: Arthralgia, myalgia, weakness.
- Renal and urinary tract: Interstitial nephritis.
- Reproductive, breast: Gynecomastia.
- General local: Discomfort, increased sweating.
