What is Gofen 400 mg used for?

Gofen 400 mg belongs to the group of non-steroidal anti-inflammatory drugs. It is a painkiller for mild to moderate pain, and a fever reducer in some cases. However, there are also some adverse effects, so its use should be under the administration of a doctor.

1. What's the effect of Gofen?

The active ingredient of Gofen 400 mg is Ibuprofen 400mg. Ibuprofen is a non-steroidal anti-inflammatory drug (NSAID), derived from propionic acid. Ibuprofen, like other non-steroidal anti-inflammatory drugs, has analgesic, antipyretic and anti-inflammatory effects. The mechanism of action of this class of drugs is to inhibit prostaglandin synthetase and prevent the production of inflammatory chemical mediators such as prostaglandins, thromboxane, and others.

The anti-inflammatory effect of Ibuprofen usually appears after 2 days of treatment. Ibuprofen also has a stronger antipyretic effect than Aspirin and lasts longer than Paracetamol, but is not used in all cases of fever.

Gofen 400 mg has a good analgesic effect in the treatment of moderate and mild arthritis pain.

2. Indications and contraindications of Gofen 400 mg

Gofen 400 mg is indicated in these cases:

  • Anti-inflammatory and relieves mild to moderate pain: Menstrual pain, headache, dental procedures, vulvar mutilation;
  • Using Ibuprofen in combination can reduce the dose of opiate drugs to treat pain after major surgery or for patients with cancer pain;
  • Treatment of rheumatoid arthritis, juvenile rheumatoid arthritis;
  • Reduce fever in some cases or when not responding to Paracetamol. 

The drug is contraindicated in these cases:

  • Hypersensitivity to Ibuprofen, Aspirin or other non-steroidal anti-inflammatory drugs. It can be detected when there are the appearance of asthma, rhinitis, urticaria, dyspnea, angioedema after taking the drug;
  • Progressive peptic ulcer;
  • Do not use to reduce fever until the cause of dengue fever cannot be ruled out;
  • Patients with asthma or bronchospasm, coagulopathy, cardiovascular disease, liver failure or severe renal failure when the glomerular filtration rate is less than 30 ml/min;
  • The patient is being treated with anticoagulants such as Coumarin;
  • Patients with congestive heart failure, reduced blood volume when taking the drug will increase the risk of kidney dysfunction;
  • Patients with colloid diseases, if taking the drug, are at risk of aseptic meningitis;
  • Pregnant women especially in the last 3 months of pregnancy;
  • Do not use it in children under 6 years of age.

3. How to use Gofen and the recommended dose

Usage: After eating, do not drink on an empty stomach because it increases the risk of side effects on the gastrointestinal tract.

Dosage:
In adults:

  • Pain relief: 1.2 - 1.8 g/day, divided into 3 doses. Maintenance dose 0.6 - 1.2 g / day when effective. If more intense pain relief is required, the dose can be increased, the maximum recommended dose is 2.4 g/day or 3.2 g/day, divided into 3 doses;
  • Fever reduction: 200 to 400 mg, every 6 to 8 hours, with a maximum dose of 1.2 g/day. 

In children over 12 years old:

  • 20-30 mg/kg/day, divided into several small doses. A maximum of 40 mg/kg/day may be given for the treatment of juvenile rheumatoid arthritis;
  • Or it can be used another way: Fever reduction: 5 - 10 mg/kg depending on the degree of fever and 10 mg/kg for pain; Each dose can be given 6-8 hours apart if needed and the maximum daily dose should not exceed 40 mg/kg. 

In patients with renal failure:

  • It is necessary to reduce the dose more than usual to avoid drug accumulation.

4. Adverse effects when using Gofen

Some common side effects such as:

  • Fever, fatigue; abdominal distension, nausea, vomiting; headache, dizziness, restlessness; and rash. 

Uncommon side effects:

  • Systemic: Allergic reactions such as bronchospasm in patients with asthma, rhinitis, and urticaria;
  • Gastrointestinal: Abdominal pain, gastrointestinal bleeding, causing a progressive peptic ulcer;
  • Central nervous system: Drowsiness, not alertness, insomnia, tinnitus;
  • Other: Visual disturbances, hearing loss, prolonged bleeding... 

Rarely, the following manifestations appear:

  • Whole body: Angioedema, rash, Stevens - Johnson syndrome, and hair loss;
  • Central nervous system: Depression, aseptic meningitis, especially those with autoimmune diseases, blurred vision, color vision disorders, and decreased vision;
  • Blood: Total leukopenia, neutropenia, eosinophilia, granulocytopenia, thrombocytopenia, and anemia;
  • Liver: Gallbladder contractile dysfunction, hepatotoxicity, and decreased liver function;
  • Urinary and genitourinary: Cystitis, hematuria, acute renal failure, interstitial nephritis, and nephrotic syndrome.

5. Precautions when using Gofen

When using Gofen 400, you should note that:

  • Before taking Gofen 400 mg, you need to carefully read the drug instructions and contraindications;
  • Caution should be exercised when administering this drug to elders, with manifestations of coagulopathy;
  • Pregnancy: Non-steroidal anti-inflammatory drugs can inhibit uterine contractions and cause delayed delivery. It can also cause severe pulmonary hypertension and the risk of severe respiratory failure in the neonate due to premature closure of the ductus arteriosus. Anti-inflammatory drugs also inhibit platelet aggregation, increasing the risk of bleeding. So do not take this medicine especially in the last 3 months and consult your doctor first;
  • Lactating women: Although Ibuprofen passes into breast milk very little, there is also a risk to the infant at doses which are normal to the mother. You still need to be careful when taking medication;
  • In case you experience some side effects, you should: If the patient sees blurred vision, decreased vision, color disturbances, the ibuprofen must be stopped. If you have mild digestive disorders, you should take the drug while eating or drinking with milk. If you find it difficult to breathe, have a rash, or urticaria, you need to stop taking the drug and go to a medical facility immediately;
  • Attention to drug interactions: Do not take this drug with other non-steroidal anti-inflammatory drugs because it increases the side effects on the gastrointestinal tract. Do not use it with quinolones as the effect on the central nervous system may lead to convulsions. If used with Methotrexate, Ibuprofen increases the toxicity of Methotrexate. Ibuprofen may reduce the sodium-reducing effect of furosemide and other diuretics. As for the drug Digoxin, Ibuprofen can increase the concentration of digoxin in the blood plasma, the dose should be adjusted accordingly.
  • Medicines can be used to reduce fever, but only when Paracetamol does not respond or respond poorly. This medicine should not be used in the first place to reduce fever.
