Panto Denk 20 is an enteric-coated tablet containing Pantoprazole 20mg. It is used to treat gastroesophageal reflux disease (GERD) and prevent the recurrence of reflux esophagitis.
1. What are the Uses of Panto Denk 20?
The active ingredient in Panto Denk 20 is Pantoprazole 20mg. This is a benzimidazole derivative that inhibits gastric acid secretion (by a specific reaction with the proton pump of parietal cells). The inhibitory effect of the drug depends on the dose and mechanism. In most patients, Panto Denk 20 reduces symptoms after about 2 weeks of treatment.
Indications for Panto Denk 20 include:
- Treatment of mild reflux and related symptoms such as heartburn, acid regurgitation, and voice loss;
- Long-term treatment and prevention of the recurrent reflux esophagitis;
- Prevention of gastrointestinal ulcers caused by NSAIDs (non-steroidal anti-inflammatory drugs) in patients at risk due to continuous NSAID treatment.
2. How to Use Panto Denk 20
Usage: Oral pathway.
Swallow the whole tablet Panto Denk 20 with a little water, do not chew or crush the tablet.
Dosage:
- For adults and children aged 12 years and older:
- Reflux disease and related symptoms: Take Panto Denk 20 at a dose of 1 tablet/day. Symptoms improve within 2-4 weeks of treatment. For esophagitis, take 1 tablet/day. Symptoms improve within 4 weeks of treatment. If the desired effect is not achieved, continue treatment for an additional 4 weeks. When symptoms are relieved, take 1 tablet/day;
- Long-term treatment and prevention of recurrent reflux esophagitis: The maintenance dose of Panto Denk 20 is 1 tablet/day. The dose can be increased to 2 tablets/day in case of recurrence. After one course of treatment at 2 tablets/day, the dose can be reduced to 1 tablet/day. For long-term treatment exceeding 1 year, a doctor's approval is required;
- Prevention of gastrointestinal ulcers caused by NSAIDs in adults: Take Panto Denk 20 at a dose of 1 tablet/day;
- Patients with liver impairment: The dose of Panto Denk 20 should not exceed 1 tablet/day;
- Patients with renal impairment and the elderly: No dose adjustment is necessary;
- Children under 12 years: Panto Denk 20 should not be used.
Note: The dosage of Panto Denk 20 is for reference only. The doctor will determine the appropriate dosage based on the patient's condition and disease progression.
Overdose: In case of overdose, the patient should undergo gastric lavage, take activated charcoal, and receive symptomatic treatment. Additionally, monitor heart function and blood pressure; if vomiting persists, monitor fluid and electrolyte balance.
Missed Dose: If a dose is missed, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose and take the next dose as scheduled.
3. Side Effects of Panto Denk 20
When using Panto Denk 20, some unwanted side effects may occur:
- Common: Fatigue, headache, dizziness, skin rash, urticaria, dry mouth, nausea, vomiting, abdominal pain, diarrhea, constipation, muscle pain, joint pain, etc.;
- Less common: Weakness, dizziness, insomnia, lightheadedness, itchy skin, elevated liver enzymes, etc.
If side effects occur, stop taking the drug and notify your doctor or go to the nearest medical facility for early examination and treatment.
4. Precautions When Using Panto Denk 20
Contraindications for Panto Denk 20 include:
- Hypersensitivity to active ingredients, peanuts, soy, or excipients of the drug;
- Do not use it concurrently with atazanavir.
Cautions when using Panto Denk 20:
- Patients with liver impairment should have liver enzymes monitored during treatment with Panto Denk 20, especially during long-term treatment. If liver enzymes increase, discontinue the drug;
- Use of Panto Denk 20 to prevent gastrointestinal ulcers caused by NSAIDs should be limited to patients with continuous NSAID treatment and those at high risk for gastrointestinal complications. Assess risks based on factors such as age (over 65 years), history of gastrointestinal ulcers, gastrointestinal bleeding, etc.;
- Panto Denk 20 can cause malabsorption of vitamin B12. Consider this issue, especially in long-term treatment patients with risk factors for B12 malabsorption or reduced B12 stores;
- Rule out the possibility of malignancy based on suspicious warning signs or gastric ulcers as the drug may mask symptoms and delay diagnosis;
- If symptoms persist despite appropriate treatment, consider checking for other health issues. Regular monitoring is needed for patients treated with Panto Denk 20 for more than 1 year. Check patients who do not respond to treatment after 4 weeks;
- Patients using the drug may be at risk of hypomagnesemia. Doctors should check magnesium levels before starting treatment and periodically during treatment;
- High-dose and long-term (over 1 year) use of Panto Denk 20 may increase the risk of hip, spine, and wrist fractures (mainly in the elderly or those with multiple risk factors). Patients at risk for osteoporosis should be carefully managed and take appropriate doses of vitamin D and calcium;
- The drug contains maltitol, so patients with hereditary fructose intolerance should not use it;
- Some side effects like blurred vision and dizziness may affect the ability to drive and operate machinery;
- Consult a doctor before using Panto Denk 20 in pregnant and breastfeeding women.
5. Drug Interactions with Panto Denk 20
Panto Denk 20 may interact with:
- Drugs that reduce the absorption of pH-dependent bioavailability: ketoconazole, itraconazole, atazanavir;
- Drugs affecting the liver enzyme system. However, clinical trials show no significant interactions between Panto Denk 20 and drugs metabolized by the same isoenzyme;
- Potential increase in INR and prothrombin time when used concurrently with Panto Denk 20 and warfarin. There is a risk of abnormal bleeding and death, so monitor these parameters when using both drugs;
- Sucralfate may slow absorption and reduce the bioavailability of Panto Denk 20. Therefore, Panto Denk 20 should be taken at least 30 minutes before sucralfate.
Using Panto Denk 20 must strictly follow the doctor's instructions to ensure effective treatment and minimize the risk of dangerous complications.
