Busulfan is an alkylating agent that inhibits DNA replication and RNA transcription, disrupts nucleic acid function, and has nonspecific effects on phases of the cell cycle. Busulfan has two unstable methanesulfonate groups attached to opposite ends of the 4-carbon alkyl chain.
Uses:
Busulfan has a selective inhibitory effect on the bone marrow. At low doses, the drug inhibits granulocytosis and, to a lesser extent, platelet aggregation, with little effect on lymphocytes. At higher doses, the drug strongly suppresses bone marrow. Because of its selective action, busulfan is used to alleviate the symptoms of chronic myeloid leukemia such as: Splenomegaly, pleasant feeling, decreased white blood cell count and increased hemoglobin. In addition, the drug Busulfan is effective in the symptomatic treatment of chronic myeloid leukemia. This is a necessary and first choice drug, used alone, or in combination with cyclophosphamide for adjuvant therapy, before allogeneic marrow transplantation in patients.
Busulfan has a half-life: 2.5 hours, bioavailability: 68-80% depending on age. The drug is metabolized by the liver and excreted in the urine (25-60%).
Indications for the treatment of Busulfan:
Used in combination therapy with cyclophosphamide or fludarabine/clofarabine as a conditioning agent before bone marrow transplantation, especially in chronic myelogenous leukemia (CML), diseases leukemias, lymphomas, and other myeloma. Busulfan may control tumor burden but does not prevent transformation or correct cytologic abnormalities. Treatment of cases such as: Bone marrow fibrosis, idiopathic erythrocytosis and thrombocytosis.

Route of administration: The drug is used through a slow intravenous infusion, at least 2 hours. Busulfan is used only as prescribed by a specialist and by qualified medical staff in the hospital.
Dosage: The dose is used between doses of the drug every 6 hours or as prescribed by the doctor.
For chronic myelogenous leukemia with allogeneic hematopoietic stem cell transplantation: In combination with cyclophosphamide prior to allogeneic hematopoietic stem cell transplantation, the recommended dose of busulfan is 0.8 mg/kg body weight, injected IV slowly, every 6 hours, for 4 consecutive days (16 times in total). Note: Before your first dose of busulfan, you need to take phenytoin to prevent seizures. Take the drug according to a certain regimen during busulfan injection. For chronic myeloid leukemia according to conventional chemotherapy: Adult dose is 4 - 8mg/day. For polycythaemia vera: The dose is 4-6mg/day continuously for 4 to 6 weeks, closely monitored for hematology and the maintenance dose is half the therapeutic dose. For cases of unexplained thrombocytopenia: dose 2-4mg/day. With myelofibrosis: The starting dose is 2-4mg/day. Maintenance dose 2-4mg/day, used every other day. Busulfan drug should not be used in the following cases:
The patient is allergic to busulfan or any of its ingredients. Chronic myeloid leukemia that was resistant to Busulfan on previous treatment. Do not use Busulfan without an accurate diagnosis.

Busulfan may interact with:
Live Adenovirus type 4, type 7 vaccines; Live vaccines Bacillus Calmette and Guerin; Vaccines for influenza, measles, mumps, rubella, chickenpox, typhoid. Ketoconazole drug; Metronidazole drug ; Thioguanine drugs when used in combination for a long time cause hepatotoxicity; The drug Itraconazole; Phenytoin drug ; Acetaminophen drug.

Common side effects when taking Busulfan:
Nausea, vomiting; Diarrhea, constipation; Anorexia; Stomach pain, abdominal pain; Dizzy; marrow failure; Leukopenia; Anemia; Pneumonia; Canker sores, inflammation of the pharynx and tongue; Insomnia, depression; Consciousness disorder.
