Is Prolonged Yellow Postpartum Discharge Dangerous?

Patient Inquiry:

Dear Doctor,

I am six months postpartum, but my lochia discharge has persisted for a long time.  The red discharge stopped after 3-4 weeks, followed by brown discharge for 2-3 months.  Now, it's a light yellow, and it continues until now without ceasing. I've also noticed that there are some ants on my underwear. I am very concerned. Due to the COVID-19 pandemic and childcare responsibilities, I have been unable to visit a clinic. So I have a question: Is prolonged yellow postpartum discharge dangerous?  Thank you for your advice.

Answer

Dear,

Lochia (postpartum discharge) is a normal physiological process in women after childbirth.  It typically lasts 2-6 weeks. The lochia is dark red in the first three days, containing blood fluid and blood clots. Then, the lochia becomes more diluted, lighter, and pinkish. After that, in 7 – 10 days of postpartum, the blood lochia includes uterine lining and cells so the lochia is yellowish–white, called alba.

Normally, lochia is free of pus. However, as it passes through the vagina, it loses its sterile nature and can become infected with bacteria such as streptococci, staphylococci, bacilli, etc.  The lochia often has a foul odor and an alkaline pH. If the lochia is infected, it will have a foul odor. Typically, lochia ceases within 20 days, though some women experience it for up to 45 days. In some cases, the discharge comes out little by little and may persist for up to two months.

Your description indicates prolonged lochia discharge.  This prolonged discharge can create a favorable environment conducive to vaginal bacterial infections, resulting in persistent yellow discharge. This is not normal. You may have acute vaginitis that requires immediate treatment to prevent serious consequences.

Therefore, regarding your question about the danger of prolonged yellow postpartum discharge, I recommend that you seek consultation from an obstetrician-gynecologist for a thorough examination to identify any abnormalities and receive appropriate treatment.  Consider consulting with a healthcare professional at a reputable medical facility.
