2. Structure of the perineum

The perineum consists of soft tissues, muscles, and ligaments that close off the bottom of the pelvic cavity. Its structure is divided into three layers: the deep, middle, and superficial layers. Each layer comprises muscles wrapped in its own fascia.

  • Deep layer: Includes the levator ani and coccygeus muscles, surrounded by two fascial layers of the deep perineum.
  • Middle layer: Contains the deep transverse perineal muscle and the external urethral sphincter, both located in the anterior perineum and enclosed by two fascial layers.
  • Superficial layer: Includes five muscles (superficial transverse perineal, bulbospongiosus, ischiocavernosus, external anal sphincter, and constrictor of the vaginal orifice). The external anal sphincter is situated in the posterior perineum, while the other four muscles are in the anterior perineum.

The perineum, as part of the reproductive system, supports and protects pelvic organs such as the uterus, vagina, rectum, and bladder. It facilitates sexual intercourse, sperm reception, and reproductive functions.
During childbirth, the perineum is especially significant as it stretches to allow the baby to pass through. 
If the perineum does not stretch adequately—particularly in first-time mothers whose perineal tissues are more rigid—it may tear or sustain injuries. These injuries not only affect appearance but may also reduce sexual satisfaction, causing pain, discomfort, and decreased libido.
