During sexual intercourse, some women experience heavy vaginal bleeding, especially after their first time. Many women have this bleeding for several days. So what causes prolonged bleeding after the first time having sex?
1.Is it normal to bleed after first time sex?
The hymen is a thin membrane located inside the vagina, about 2-3 cm from the vaginal opening, and acts as a barrier to it. The shape and thickness of the hymen vary from person to person.
In very rare cases, some women are born without a hymen while others have a hymen that is too thin and easily ruptured due to various causes such as falls, accidents, or vigorous exercise. Conversely, in cases where the hymen is too thick, it may remain intact even after multiple sexual experiences or even childbirth.
When the hymen is ruptured, women may experience:
- Bleeding after first time sex: The hymen is a thin membrane, so when it ruptures, women often see a small amount of bright red blood on their underwear or mixed with semen.
- Pain: This feeling of pain will be more pronounced in women who experience hymen rupture after their first sexual intercourse. Depending on the specific case and each person's body, the pain threshold will also be different. However, women should not be too worried because this pain will quickly go away after a few days.
2. How long does bleeding after first time sex last?
Regarding the duration of bleeding after the first time having sex, gynecologists say that the amount of blood from the hymen varies depending on each person's body. However, usually, after the first time having sex, there will only be a few drops of blood and it will dry up quickly. Therefore, women do not need to worry about this.
3. Causes of Excessive Bleeding After First Time Sex
However, some women experience prolonged bleeding after sexual intercourse due to factors such as strong force causing damage to the area, improper techniques, or the use of sex toys. These agents can cause injuries at any point in the reproductive system, such as tears, lacerations, or perforations, resulting in excessive bleeding.
Common causes of prolonged bleeding after first-time sex include:
- Tears in the hymen at a location with many blood vessels, occurring in cases of first-time intercourse with excessive force.
- Tears in the vaginal wall.
- Tears in the perineum (The perineum is the area between the anus and the vulva in females).
- Severe tears in the perineum and perineal pouch, penetrating into the abdominal cavity due to rough movements.
- Unsafe sexual positions that are incompatible with the female anatomy or the use of sex toys with attachments such as metal balls or spikes.
4. Treatment for Excessive Bleeding After First Time Sex
Immediately after sexual intercourse, if you experience excessive bleeding, you should go to a medical facility or gynecology clinic. A doctor will examine and assess the extent of the injury to determine the appropriate treatment.
In cases of a torn hymen with active bleeding or a tear in the outer vaginal wall, the doctor may administer a local anesthetic like 2% Lidocaine and then suture the bleeding site with 2.0 chromic suture.
In cases of deep tears in the vaginal wall, accompanied by tears in the perineum, the doctor may administer pre-anesthesia with Atropine 0.25mg or Dolargan 100mg and suture the tear site with 2.0 chromic suture or 2.0 Vicryl suture to control bleeding.
In cases of excessive bleeding causing a drop in blood pressure, the patient may need fluid resuscitation with isotonic saline such as 0.9% Sodium Chloride, combined with a blood transfusion with the same blood group. Afterward, the patient will be transferred to an operating room for further treatment. In these cases, the injury is deep and complex, causing significant blood loss. When examining the vagina, there may be complex injuries, with the tear extending deep into the abdominal cavity, requiring laparoscopic surgery to suture and control bleeding. After managing the bleeding site, close monitoring is necessary, along with systemic antibiotics to prevent infection, such as Augmentin, Cephalexin, or Negacef. For mild cases with minor injuries, intramuscular antibiotics like Zadim, Cefuroxim, or Alfacef can be used in combination with pain relievers such as Paracetamol or Efferalgan, and disinfection with Betadine solution.
5. How to Prevent Excessive Bleeding After First Time Sex
Here are some ways to prevent prolonged bleeding after first-time sex:
Prior communication: Before engaging in sexual activity, both partners should discuss and agree on a pace and style that is comfortable for both.- Gentle and gradual approach: Initial movements should be gentle and soft, avoiding forceful or rough actions.
- Avoid uncomfortable positions: Do not attempt positions that cause discomfort for either partner.
- Use lubricant: For older women, using a lubricant can help prevent dryness and reduce the risk of vaginal tissue damage.
- Avoid sex toys: Absolutely avoid using sex toys or homemade devices, as they can cause injuries and infections.
Safe and proper sexual practices can minimize the risk of injury to both male and female reproductive organs. This approach can also help prevent prolonged bleeding after first time having sex.
