Enhanced Early Recovery Approach (ERAS)
ERAS are methods of patient care before, during and after surgery with the participation of many specialties in the hospital to help patients recover soon after surgery. The goal of ERAS is to provide comprehensive patient care in the surgical period to reduce hospital stay, improve treatment quality and reduce costs; reduce the complication rate for the patient. ERAS helps to reduce the time of care by 30%, reducing complications after surgery by 50%.
The effect has been "measured" very specifically. Statistics in New Zealand, each case of ERAS reduces NZ$4,000, in Switzerland it saves 1,500 euros, in Canada, it saves 2,200-2,900 on rectal and esophageal surgery. The average cost of surgery for the patient decreased from 2,800 to 5,900 USD, the hospital stay decreased from 6 days to 4.5 days.
Because it brings many benefits to both patients and hospitals, this method has so far been applied by 100 hospitals in 20 countries around the world. However, compared with traditional post-operative care, ERAS is so different that it may seem odd at first glance and requires the coordination of many specialties and the compliance of the staff to the procedure. medical staff. Therefore, ERAS is also a challenge for hospitals.
Approaching ERAS since 2017, Vinmec has officially applied this method since May 2018 for patients undergoing rectal surgery and became the first hospital to apply ERAS in Vietnam.
After nearly 1 year of application, up to now, there have been 11 cases of colorectal surgery being cared for by ERAS. Most patients were discharged after 3-5 days after surgery, instead of having to stay in the hospital for 7-10 days as before. The rate of complications, length of hospital stay as well as the cost of the patient are reduced, bringing many benefits to both the patient and the hospital.
At Vinmec, ERAS implementation is coordinated by many faculties/departments: Anesthesiology, Nutrition, Rehabilitation, Clinical Pharmacy, Surgery and Nursing care with many personnel.
Colon cancer surgery patients apply ERAS without fasting, no enema. During surgery without drainage, optimal infusion to avoid water-electrolyte disturbances, pain relief without opioid preparations, antibiotics with only 1 prophylactic dose, saving costs, and increasing effectiveness against infections. After surgery, during the day, drink water, milk, eat soft, sit up for 2 hours; The first day after surgery is to give up fluids and eat raw, practice walking for 6 hours. The basis of maintaining the patient's activities that do not differ greatly from the normal circadian rhythm of each person will reduce symptoms and organ dysfunction after surgery. Therefore, the patient will recover soon after surgery.
The goal is to leave the hospital on the 3rd day after surgery. And in fact, on the 2nd day, the patient was able to walk on his own and return to bowel circulation on the day of surgery or the day after surgery. With major surgery such as colectomy, if before, the patient had to stay in the hospital for 7-10 days with daily infusion of antibiotics, erratic drainage, but now eat and drink as usual, without drainage, without infusion. , 3 days is discharged with good health.
